Eden Hazard's lone goal was enough for Chelsea to win against Pardew's Crystal Palace as well as secure their 5th PL title. The 'Special One', Jose Mourinho, beat all odds and lead the Blue's to their 4th title in 11years. Let's have a look at their journey.

Just four matches into the season, and Costa had his hat-trick. Loic Remy hit a fourth to ensure Chelsea walked out with all three points. Chelsea won 4-2.

Mourinho's men hosted Arsenal in Stamford Bridge in one of the most heated encounter. Wenger had never been able to win against his Portuguese counterpart. His frustration was visible when he pushed Mourinho on the sidelines post a 2-0 defeat of Arsenal.

Papiss Cisse dented Chelsea's hopes of having an undefeated season when he hit a brace against them. Drogba managed to grab one for the Blues, but it was too late by then. Chelsea lost 2-1.

Chelsea's visit to White Hart Lane was a one to forget when they were humiliated 5-3 by hosts. Their defense was exposed for the first time to such an extent. Teen sensation Harry Kane hit a brace.

Though Chelsea won this match 2-1, it will remembered for Charlie Adam's 45-yard screamer. The Stoke City midfielder, caught Courtois off his lines and had a go on goal. He equalised just before half-tine.

The reverse fixture of this game saw United clinch an extra-time equaliser. But this time around, Louis van Gaal's men could not exploit Chelsea defense and were outclassed by Eden Hazard's lone goal.

Post a goal less draw with Arsenal, Chelsea needed to win this match against Crystal Palace to clinch their 4th title in 11 years. And who else than Eden Hazard, ensured his side got exactly what they wanted when he led his side to the PL title by scoring the only goal.
