Ranbir has had a nasal septum issue since he was a child. This has caused his nose bone to be slightly crooked. Despite his condition, Ranbir has chosen not to undergo corrective surgery. In 2009, he needed a straighter nose for a movie role, and the director had suggested surgery. But Ranbir firmly declined the idea.

According to India Forums, a close friend of Ranbir revealed, "Ranbir has been facing this nose issue since he was a kid. Doctors who specialize in ear, nose, and throat (ENT) matters have suggested a procedure called septoplasty, which could help. However, Ranbir has consistently refused this option. He has always been quite determined, and once he makes up his mind about something, he sticks to it."

The friend also shared, "Even his parents, Rishi Kapoor and Neetu Kapoor, tried persuading him multiple times to consider a minor nose surgery. Eventually, they gave up. Ranbir isn't opposed to the idea of surgery itself, but he is concerned about altering the shape of his nose. He once mentioned to me, 'I would rather keep my crooked nose than have it reshaped.'"
